Dr. David Samadi of the Fox News Medical "A-Team" has performed Israel's first ever "SMART Surgery" robotic prostate removal surgery at Rambam Hospital in Haifa, with three surgeries performed in total over the past three days.

Robotic prostate removal has several advantages over traditional prostate removal surgery including less blood loss, a shorter hospital stay, and a better post-operative quality of life regarding urinary control and sexual function.

Dr. Samadi says that the learning curve for robotic prostate surgery is steep, and that he's excited to have the opportunity to share his knowledge and to learn from the Israeli team in order to better serve patients.

Over the next few days Dr. Samadi will be performing live surgeries in Tel-Aviv and demonstrating his high-tech technique called SMART surgery to other doctors. SMART surgery aims to preserve the nerves around the prostate relating to sexual function. The surgeries are performed in an hour and a half and will be telecast to doctors in Israel.

Dr. Samadi believes that the future of robotic surgery lies in teleconferencing and telementoring to share techniques and experience between doctors around the world. He says that the concept of telesurgery where operations can be performed on patients over long distances could one day be a reality.

Dr. Samadi has been collaborating closely with Dr. Jack Baniel of Rabin Medical Center and Dr. Jacob Ramon of Tel Hashomer Hospital to mentor fellow colleagues in Israel on his SMART Surgery Technique.

More information on Dr. Samadi's SMART surgery technique can be found at:

The following video highlights the benefits and techniques of the newest generation 180W Greenlight XPS laser system for the management of male BPH (benign prostatic hyperplasia). Previously, TURP, a procedure in which an electrical loop is used to cut specimens of the prostate (with risks of 4-8% transfusions, electrolyte anomalies, and prolonged hospitalization of 2-3 days) however is limited to smaller prostates less than 100g and men who are not taking any anticoagulation medications. The Greenlight laser has the advantages of little to no bleeding, no electrolyte anomalies since normal saline is used for irrigation, and is performed as an outpatient surgery. The procedure can also be offered to men with large prostates (greater than 100g), men taking blood thinner medications and those with significant comorbidities. The 3rd generation of Greenlight XPS (AMS) offers the greatest strength laser power at 180W and significantly reduces 40-50% operative times. We are proud to offer our patients the surgical management of BPH care.

Your doctor has recommended that you undergo a Trans Urethral Resection of the Prostate – or TURP. But what exactly does that mean?

The prostate gland is a walnut-sized organ that is part of your reproductive system.

It provides some of the fluid contained in semen.

The prostate is located just under the bladder and behind the testicles.
The urethra — a hollow tube that carries both urine and semen to the penis — passes through the prostate.

In some men, the prostate gland becomes enlarged. Symptoms of an enlarged prostate include:
Patient Education
* a full bladder feeling even when the bladder is empty

* pain when urinating

* weak urinary stream

* infertility

* and sexual dysfunction.

To relieve you of your symptoms, your doctor feels that you would benefit from a surgical procedure called TURP.

TURP is designed to relieve symptoms by reducing the size of the prostate.
It is also a diagnostic procedure. Tissue removed during a resection of the prostate or TURP is routinely screened for the presence of cancer.

So make sure that you ask your doctor to carefully explain the reasons behind this recommendation.

Your Procedure:

On the day of your operation, you will be asked to put on a surgical gown.

You may receive a sedative by mouth and an intravenous line may be put in.

Once on the table, your feet and legs will be placed in an elevated position with your knees apart.

The nurse will swab the penis with an antiseptic solution.

Your doctor will then lift your penis upward.

A well-lubricated instrument called a resectoscope is then gently inserted into the urethra.

When the resectoscope reaches the back of the penis, your doctor will pull the penis downward in order to create a straight path into the prostate.

Using this tool, your doctor will then scrape excess tissue from the prostate, restoring it to its normal size.

Tissue removed from the prostate may be sent a laboratory for analysis.

When the surgery is complete, your doctor will remove the resectoscope. Your doctor will probably ask you to wear a temporary Foley catheter.

A Foley catheter is a narrow tube inserted through your urethra and into your bladder. The catheter is connected to a bag that is attached to your leg by a strap. While the Foley catheter is in place, urine will pass from your bladder into the bag. You will not need to urinate into a toilet.

The nurse will show you how to change the bag when it is full. An appointment will be made for you to return to the doctor's office in a couple of days to have the catheter removed. As soon as the anesthesia wears off and you feel comfortable, you'll be allowed to leave.

What is BPH?

The gradual enlargement of the prostate is a non-cancerous disease common among aging men. The enlarged prostate can intrude into the urethra causing partial or complete obstruction of urine flow. This condition is referred to as benign prostatic hyperplasia, or BPH. While BPH is usually not life threatening, it can cause considerable discomfort, with complete obstruction leading to emergency surgery for resolution. Over half of all men over 50 have BPH symptoms, with the percentage growing to 80% of men over 80. Due to the overall aging of society, the number of men seeking treatment over the next 20 years is expected to increase.
Why Holmium?

Tissue Removal
The holmium laser is ideal for precisely and cleanly ablating without charring or overheating tissue. Since the holmium absorption depth is only .5 mm or less, tissue is ablated faster than heat is conducted into surrounding tissue. Holmium energy is delivered in a pulsed mode with each pulse containing enough energy to vaporize the tissue. The vaporized tissue is removed before it can conduct heat, which minimizes thermal damage. The resulting what-you-see-is-what-you-get effect contrasts to the coagulating effects of Nd:YAG and KTP lasers whose wavelengths penetrate deeper and extend below the visible tissue surface.

Hemostasis
Hemostasis with holmium is remarkably good. Its localized coagulation effect seals the tissue and provides hemostasis superior to electrocautery instruments without producing deeper thermal injury associated with Nd:YAG or KTP lasers. The holmium coagulating effect minimizes fluid absorption better than electrocautery.

The ablating and hemostasis features of holmium make it an excellent energy choice for BPH treatment. When stone fragmentation benefits are considered, the holmium laser is the best and most cost effective choice for treating both stone disease and BPH—two of the most common reasons patients visit the urologist.

Holmium Prostate Treatments
Since its introduction, holmium laser prostatectomy has undergone rigorous scientific studies that substantiate its surgical equivalence to the traditional electrocautery procedure (TURP) with fewer patient complications and faster patient return to normal activities. Whether performing holmium laser enucleation (HoLEP) or holmium laser ablation (HoLAP) of the prostate, the procedure can be done as a day case or in a 23-hour facility. HoLEP and HoLAP treatments offer many benefits including:

* Immediate removal of tissue and symptoms.
* Virtually bloodless and minimally invasive technique.
* Short catheterization time.
* Few complications.
* Short convalescence.
* Cost-effective solutions—hospitals gain unmatched economic benefits by utilizing the multipurpose holmium laser for stones, strictures, tumors AND BPH treatment.
